Michael’s Angel Paws Activities - Michael’s Angel Paws, Inc. (“MAPs”) asks you to sign this form as a pre-condition to you and/or your child participating in a MAPs activity. (This form is intended to cover all MAPs activities—including, for example, you and/or your child acting as a canine coach, providing dog training services to the public, training and caring for a therapy dog, working a MAPs event (including therapy events), or so on.) Your participation in such activities is voluntary, not mandatory. By signing below, you agree to all of the following:

Assumption of Risks - MAPs is committed to the safety of everyone who participates in events and or training sessions. However, you and/or your child’s participation in a MAPs activity/training sessions, particularly your interaction with any animals, has certain inherent risks, including potential risks to your health, safety, and well-being and those of your pet(s). You understand these risks and have had the opportunity to discuss them with MAPs and others. You assume all risks involved in you and/or your child participating in the MAPs activity, including those arising due to anyone’s carelessness. Please exercise caution!

Liability Waiver - You agree (for yourself, your family members, and your pet(s)):

to waive, release, and discharge MAPs and its directors, officers, employees, contractors, volunteers, and agents (collectively, the “Releasees”) from all liabilities (including personal injury, disability, death, property damage, property theft, and similar losses or harms) and claims (of any kind, including in law or equity, contract or tort, and existing or contingent) relating to or arising out of you and/or your child participating in the MAPs event, including as a result of a Releasee’s negligence, and you covenant not to sue a Releasee for such liabilities or claims;

to indemnify and hold harmless the Releasees from such liabilities and claims, including reasonable attorneys’ fees and costs (whether or not a legal proceeding commences);

MAPs are not responsible for the acts or omissions of third parties, including other participants; and

MAPs are entitled to all of the protections in 24-29 of S.B. 4 (2020) with respect to COVID-19 risks.

Media Release - The MAPs activity may be photographed and/or recorded. By participating, you are permitting MAPs to use the likeness of you and/or your child, and your dog(s), in photographs, audio recordings, video recordings, and/or other digital reproduction formats or media, and all tangible and intangible publications—including website entries, radio/TV/news spots, and social media—without payment, royalties, or any other compensation.

Refund Policy - All group class sessions (Virtual/In- Person) are non-refundable; unsatisfactory or complaints will be addressed on a case-by-case basis by our chief executive officer at info@michaelsangelpaws.org

Cancellation Policy/Make-up Policy – You require a 24hr notice if you cannot attend a session. If no notification is given to the canine coach, it will be counted as a missed session and a make-up session will not be offered. However, if you have notified your canine coach, we can arrange to meet with you 15min before for review session.
Complaint Policy for Non-employees -
Any non-employee (including volunteers, students, recipients, or donors) who has a grievance concerning Michael’s Angel Paws has the right to file a complaint in accordance with the procedures outlined in this policy.
Grievances may include, but are not limited to, perceived violations of rights, concerns regarding the quality of services, or discrimination based on race, age, disability, or any other protected category.
All complaints will be treated with dignity and respect, regardless of their nature. Individuals will not be discriminated against, harassed, intimidated, or subjected to any form of retaliation for filing a complaint or participating in an investigation.
If an individual feels they are being subjected to any of the above, they have the right to appeal directly to the Chief Executive Officer (CEO). Affected parties are encouraged to attempt to resolve the matter informally whenever possible.
If the issue remains unresolved, a formal written complaint should be submitted to the Founder, who will report it directly to the Executive Director. Complaints concerning the Executive Director should first be brought to their attention and, if unresolved, escalated to the President of the Board of Directors.
All formal complaints and responses will be documented and maintained on file. Complaints will be handled promptly, with the goal of resolution within 20 working days from the date of initiation. If additional time is required, all involved parties will be notified accordingly.
